How to Start Services
In order to qualify for home health services, you or your loved one must have a condition or disability that precludes the patient from leaving the home or that requires a great deal of physical assistance.
A family doctor or specialist needs to approve home health services. For more information or to send a referral, please call 1-855-843-8627.
Physical, Occupational and Speech Therapies
Physical Therapy evaluates and treats dysfunctions of movement. A Tower Health at Home PT can restore or improve physical and functional abilities, whether the patient’s issue is concerning an injury or disease. Specialized programs may include Electric Stimulation and/or Ultrasound to assist with blood flow and healing.
Occupational Therapy helps individuals achieve independence in everyday functions such as bathing, dressing, grooming, and mobility as it relates to these tasks. OTs develop alternative skills to help patients reach their priorities and personal goals. Specialized services may include a Home Safety Evaluation, adaptive equipment training, and family member/caregiver training and guidance.
Speech/Language Pathologists evaluate and treat speech and swallowing disorders. They also provide voice therapy, cognitive therapy, aural rehabilitation for the hearing impaired, as well as training in the use of alternative communication devices. Another specialized service is Vital Stimulation Therapy, which provides external neuromuscular electrical stimulation to the neck area, in conjunction with functional swallowing exercises provided by the Pathologist.
Our primary nursing staff at Tower Health at Home provide comprehensive physical, social and environmental assessments, case management, care plan development, individualized teaching and treatment programs, and discharge planning, all while providing caregiver support and education.
