Tower Health Urgent Care makes registration easy for first-time patients

Below are all the forms you will need to register as a first-time patient, complete a workers' compensation request, and authorize transfer of your medical records. 

Important Forms

  • Patient Registration[PDF]

    Complete the indicated fields on the form below and present the completed form to our receptionist upon arrival. If you have questions about your registration form, the receptionist will be able to assist you.
  • Employer Authorization[PDF]

    Workers Compensation packets must be completed for each new injury.
  • PHI Release Authorization[PDF]

    Tower Health Urgent Care requires a Personal Healthcare Information (PHI) Release Authorization to transfer your medical records. If you need to transfer medical records, please complete the PHI Release Authorization form and fax it to any one of our locations. (You can find the fax number on each individual location page.)

Tower Health Urgent Care accepts most major insurance plans and we will file all claims on your behalf.

Tower Health is in-network with nearly 60 different insurance plans. All of our in-network affiliations are listed by state below. When you arrive, remember to bring a valid photo ID and your insurance card. You will be responsible for your copay or deductible at the time of service.

For your specific copay, coinsurance, or deductible information, please contact your insurance plan’s member services. Due to the variation in plans, our billing staff cannot tell you the exact amount of your out-of-pocket cost. Out-of-pocket cost is dictated solely by your insurance plan.
