Montgomery County Street Medicine brings vital healthcare services to individuals experiencing homelessness across Montgomery County, PA. This collaborative program is implemented by Tower Health Medical Group and Access Services with additional healthcare partners including Community Health and Dental Care, and Delaware Valley Community Health. 

Street Medicine Services

  • Connection to Access Services Street Medicine Program
  • Connection to primary care through CHDC or DVHC
  • Dentistry (as needed)
  • Flu and tetanus vaccines
  • Follow-up care for recently hospitalized patients
  • Lab draws
  • Podiatry (as needed)
  • Psychiatry and mental health
  • Urgent care
  • Wound care
