Community Health Needs Assessment

Community Health Needs Assessments are completed every three years as a result of the Patient Protection and Affordable Care Act. These assessments create important opportunities to improve the health of our communities.

YouthGrow CSA

Since 2020, Pottstown Hospital – Tower Health and GreenAllies have partnered to implement a 100% subsidized Community Supported Agriculture (CSA) program that works to increase access to healthy foods for our community members who are in need. The 1-acre farm is located right in Pottstown, PA and is primarily operated by high school and college students. All produce is organically grown and harvested weekly to ensure fresh, delicious produce! 

Social Determinant of Health Screenings & Navigation

Overall health and wellness is largely shaped by where you live, work, and play. Pottstown Hospital is dedicated to addressing the Social Determinants of Health (SDoH) and works to connect patients to resources and opportunities in their communities to improve their quality of life.
