Patients emerging from acute care benefit from our Transitional Care services. You may utilize Transitional Care if you are recovering from one of the following conditions:
Cancer
Cardiopulmonary conditions
Generalized weakness
Multiple trauma
Neurological or neuromuscular disorders
Orthopedic conditions, including joint repair or replacement
Stroke or aneurysm
Surgery
Transitional Sub-acute Unit
Here we provide skilled medical and rehabilitation services that act as a transition between acute care and home. Internal Medicine physicians staff the unit, with consultations to physiatrists, as needed.
Admission Requirements
In order to be admitted to the Transitional Care Center, the patient must meet all of the following requirements:
Need daily skilled nursing care and/or specialized therapy
Have had a prior acute hospitalization of 72 hours or more (unless waived by payor source)
Be medically stable
Require inpatient services, rather than home health or outpatient services
Be willing to follow the discharge plan developed jointly by the interdisciplinary team and the patient
