Montgomery County Street Medicine brings vital healthcare services to individuals experiencing homelessness across Montgomery County, PA. This collaborative program is implemented by Tower Health Medical Group and Access Services with additional healthcare partners including Community Health and Dental Care, and Delaware Valley Community Health.
Services Include:
- Follow-up and care coordination for recently hospitalized patients
- Urgent Care
- Wound Care
- Psychiatrist evaluations and medication management
- Lab draws
- Administration of flu vaccines and tetanus shots
- Connection to primary care
- Enrollment in Access Services’ Street Outreach Program
- Enrollment in housing programs or other social services
